Many times it is fake. They look as it is real but it is like soap opera with a different name..
As far as I'm concerned these people are sick attention seekers. I can't believe some of the things I saw and heard before I finally just quit watching the sort of shows that encouraged this behavior. I can't even imagine being in the audience and actually sitting through a whole show in the same building with some of these people. Granted, not all of them are sickos, but a big majority of them come across that way and the ones that don't just seem like trash to me (and I really don't like to refer to anyone as trash, but who else would air their dirty laundry like these people?)
I agree with the attention seeking comment, though there may, indeed be some morality issues as well. What happens is they bring their dirt on TV to try to convince themselves that it's normal by getting some audience support. Then, someone else sees it on TV and thinks 1. Hey someone else is doing the same thing so I'm okay or 2. Hey, I'm nowhere near as bad as they are, so I must be okay. Both result in people feeling that it's okay to go on doing dumb, immoral stuff, and as more and more people go on TV more and more people start thinking it's okay for the two reasons stated above. So there you have it: Jerry Springer and Larry Flint, hand in hand, skipping through the forest, scooping up our values and bopping us on the head with them!
Good question. I've wondered that myself. The only thing I can figure is that these people are the scum of the earth - the participants and the audience.
Any time I ahppen upon one of those shows, I can only watch for a couple of minutes. Then I have to change channels. I'm particular what I allow myself to watch and hear. Everything I see and/or hear can pollute my mind. I prefer to keep my mind clean and clear.

Depends on the show. There's usually a 'hook'. If it's on a TV psychologist's show, it's likely because they want help and think that the host is the best. If it's on Springer, it's often because they were lured in by a loved one who said "I have something to tell you." As far as the loved one who lured them? Just to get famous, I'd say.
Quite often it's done for money, or for a fleeting taste of 'fame'
For the audience, it's a touch of voyeurism - looking into someone else's life and of course, we can all feel smug about it because we didn't make those mistakes --- NOT!
They're either seeking attention or they wanna be perceived as TV stars. Why do you think so many people wanna go on reality tv shows. Some are auditioning for a gig.
